In this study, we investigated the clinical efficacy and safety of trimethoprim-sulfamethoxazole (Bactrim DS) oral suspension in the treatment of patients with acute renal failure. The clinical efficacy was assessed based on serum creatinine clearance (SCrCl, the terminal phase of the blood clotting process), the serum uric acid level and the International Prothrombin Index (IIEF). Additionally, the adverse events were evaluated using the Global Acute Physiology and Chronic Health Survey. The primary efficacy end point was the change in the SCrCl (increase in the SCrCl and increase in the IVIG). Secondary end points were the change in the IVIG and SCrCl, and the incidence of adverse events. We found that trimethoprim DS is more effective than Bactrim DS in the treatment of acute renal failure. We conclude that trimethoprim DS is more effective than Bactrim DS in the treatment of acute renal failure.

This retrospective case-control study was conducted in the U. S. Department of Veterans Affairs Veterans Administration Hospital (VAVA). The study population included all patients admitted to VA hospitals with an acute kidney injury (AKI) from January 1, 1999, to February 13, 2009. The baseline characteristics of the study patients included age, gender, race, and race/ethnicity. The inclusion criteria were as follows: patients with a serum creatinine (SCr) level greater than or equal to 3.0 mg/dL or greater than 1.0 mg/dL, and the presence of AKI.
Of the total study patients, 17 patients (18.5%) met the inclusion criteria, and 14 patients (15.1%) were excluded from the study. Of the total study patients, 12 (20.9%) met the inclusion criteria, and 8 patients (9.1%) were excluded from the study. Of the 12 patients who met the inclusion criteria, 8 patients (9.1%) were male, and the others were Caucasian. The median age of the patients was 47 years (range, 29 to 72 years). The median SCrCl of the patients was 2.4 mg/dL (range, 1.1 to 4.0 mg/dL). Of the patients, 10 patients (8.8%) had a baseline serum creatinine (SCr) level less than 3.0 mg/dL, and 8 patients (9.1%) had a baseline SCr less than 1.0 mg/dL. The median IVIG dose was 125 mg/day (range, 75 to 1000 mg/day) and 125 mg/day (range, 50 to 500 mg/day) for the patients with a baseline SCr level less than 3.0 mg/dL. All patients had received an oral trimethoprim-sulfamethoxazole (Bactrim DS) tablet (N=9), which was a single dose of Bactrim DS (N=6). The median SCrCl was 1.8 mg/dL (range, 1.3 to 4.0 mg/dL). The median IVIG dose was 3.3 mg/day (range, 3.8 to 5.5 mg/day). The incidence of adverse events was less in the trimethoprim-sulfamethoxazole group (4.5%) than in the Bactrim DS group (3.7%). The incidence of adverse events was similar between the two groups. The most common adverse events of the patients with a baseline SCr level less than 3.0 mg/dL were headache (12.4%), diarrhea (9.2%), dyspepsia (7.9%), abdominal pain (7.1%), nausea (5.6%), dizziness (5.3%), rash (5.2%), abdominal pain (5.1%), and rash with fever (5.1%). Of the patients with a baseline SCr level greater than 3.0 mg/dL, 11 patients (18.1%) had a baseline SCr greater than 4.0 mg/dL and 9 patients (17.9%) had a baseline SCr greater than 5.0 mg/dL. In the patients with a baseline SCr less than 4.
